Continuing Education Units (CEU)
Pending approval, Continuing Education Collaborative continuing education hours will be provided for all certificate programs, mini-courses and lectures offered by the SSW. For your convenience all participants will receive CEU Request Forms. Attendance for the duration of the program is mandatory and you must sign in at the beginning and then sign out and the end of the program. You will also need to provide your state licensure number and complete an evaluation form. You will then need to provide us with a completed CEU Request Form along with a $10.00 administrative fee after the program is completed. All of the documents will be kept on file for at least five (5) years for auditing purposes.
